Precautions 

Aciclovir Prophylaxis:
400mg- BD
Treatment:
 800mg- 5 times a day- 14 days
IV- HSV- 500mg- TDS
Prophylaxis->2yrs- 200mg-BD
Treatment:
20mg/kg- 5times a day
Inj. 10mg/kg-TDS- IV
 Renal correction, avoid with amphotericin B, CSA, 
Monitor RFT
Good hydration to avoid crystal nephropathy
Avoid rapid infusion- Give over 2hrs
IV- Watch for neurotoxicity
Valaciclovir Prophylaxis- Routine- 500mg- BD
For HSCT pts(CMV prophylaxis)- 2gm, PO- QID
  
 Ganciclovir 5mg/kg- BD- IV
for 14-21 days then OD
Slow over 1 hr
Prophylaxis-
IV- 5mg/kg- 5 days a week from engraftment till 100 days of transplant
 5mg/kg- BD Renal correction, watch for cytopenia, CNS side effects, neuropathy, Hold if ANC <500, PL <25k, Adequate hydration
W/F Retinal detachment (Ophthalmoscopy 6 weekly)
Avoid with imipenem/cilastatin
Use contraception during and after 90days after treatment
 Valganciclovir Prophylaxis: 900mg- OD-PO 
Therapy: 900mg- BD-PO
  May cause BM suppression
 Entecavir 0.5mg-OD-PO  
 Oseltamivir Prophylaxis: 75mg-OD-PO
Treatment: 75mg-BD-PO
  May cause nausea
